If you have a mutilated passport, you must come to the Consulate
in person in order to obtain a new one. Only citizens living
outside extended San Francisco Bay Area may forward this request
by mail.
Be advised, the issuance of a new passport after one has been
mutilated is at the discretion of the Consul and may demand
an approval from the Ministry of Interior in Israel.
Please provide the following:
- Declaration
of Destruction of Passport Form - completed and signed
- Request
for Passport Renewal Form - completed and signed
- Valid Identification (US passport, driver's license, etc.)
- 2 passport
photographs [2" x 2"]
- $100.00 cash or money order
Further, Citizens living outside the extended San Francisco
Bay Area must sign all forms in front of a notary and provide
notarized copies of valid identification.
