The child must be registered as a citizen, must possess an Israeli passport when traveling to Israel, and must arrange his/her army status once sixteen and a half years old.

Note
: Only a parent who is an Israeli citizen can sign the request forms for the passport.

The following must be provided:

  1. Original Birth Certificate authenticated by an Apostille
  2. Notification of Birth Form
Note: A notification of birth must be made by a parent who is an Israeli citizen. On the basis of this notification an Israeli ID number will be issued to the child.
  3. Passport Request Form
  4. $17.00 cash or money order if the child is under 18 and $50.00 cash or money order for a person requesting a first passport who is over 18.

  6. 2 passport photographs [2" x 2"]

Note: If the child is under 18 then both parents must sign in front of the Consul or in front of a Notary. A passport will not be issued without the approval of both parents.
